Urgent Placement Support
Even with the best of planning, some families find themselves in need of an urgent placement.
Your loved one may be facing discharge from hospital in a number of days and due to the severity of the need, there is no option of a Transition Care Program. Your loved one cannot return home/ or if there is no appropriate support available at home. Or there can be a myriad of other reasons that has led to what seems to be an almost unbearable situation.
Who can you turn to for practical support?
1. Arrange an ACAT whilst still in the hospital.
2. Talk to the Social Worker in the hospital. She will help you with the forms.
3. Ring My Aged Care line on 1800 200 422. They will ask your details and tell you of vacancies in your preferred area.
4. Hopefully you have already filled out your Health Care Directive (click here for the form in Queensland).
